Gordon H. Sasaki, M.D., F.A.C.S. - Areas of Expertise

Dr. Sasaki has dedicated his knowledge and skills throughout his career not only to benefit his patients, but also to educate his plastic surgical peers throughout the world. His passion is to innovate and to teach new and established techniques, to embrace contemporary technologies, and to provide validated clinical research for the advancement of plastic surgery. Dr. Sasaki is frequently requested to teach and share his expertise on the following subjects at national and international plastic surgery societal meetings:


Tissue Expansion

Dr. Sasaki is one of the developers of this technique and performed the facial expansion reconstruction on “Baby Jessica.”

Since 1982, Dr. Sasaki advanced the technique from its modern clinical beginning to its full development by investigative research, clinical practice as a director of national and international meetings.

Dr. Sasaki is one of the developers of this technique (1982) that has become a an integral part of reconstructive plastic surgery (60% of breast reconstruction). He performed the tissue expansion facial surgery on Jessica McClure, the baby who was trapped inside the well in Midland, Texas. His innovations and experiences led him to be the faculty-chairman for over 40 national and international symposia that resulted in the publication of his book, Tissue Expansion in Reconstructive and Aesthetic Surgery (1998). It remains a source book on this subject.


Endoscopic Surgery - Copyright by Dr. Sasaki

Since 1993, Dr. Sasaki has advanced the development of minimally-invasive endoscopic surgery for aesthetic and reconstructive procedures.

Dr. Sasaki is considered to be one of the innovators of this minimally-invasive technique (1993) that has become a standard aesthetic (endoscopic forehead and face lifts) and reconstructive (harvesting muscle-skin flaps) procedure in plastic surgery. Through small incisions within the frontal hairline (endoscopic forehead lift) or within the temporal hairline (endoscopic face lift), a complete release of restraining structures holding the brows down effectively permits fixation of the elevated tissues without skin removal. He has taught over 20 symposia at the national and international levels that resulted in the eventual publication of his book, Endoscopic Aesthetic and Reconstructive Surgery (1996). For the last 15 years, Dr. Sasaki continues to teach this course at the American Society for Aesthetic Plastic Surgery annual meetings. Lasers

Since 1996, Dr. Sasaki has advanced the clinical usage of lasers in plastic surgery for aesthetic and reconstructive procedures and has received certification by the American Board of Laser Surgery.

Fractional Laser Device

Since 2000, Dr. Sasaki has advanced the clinical usage of lasers in plastic surgery for aesthetic and reconstructive procedures. He has given over 15 instructional courses and five teaching videos on the CO2, Erbium, and Combined Wavelength Lasers at the annual meetings of the American Society for Aesthetic Plastic Surgery. In 2008, he presented courses on the Fractional Superficial and Deep CO2 Lumenis® Systems for skin resurfacing of the face, neck , décolletage, arms, and hands to improve wrinkles, folds, solar pigmentations, and scars. Other laser usages taught involve cutting modalities (facelifts, eyelid surgeries, endoscopic forehead, and face lifts) and removal of hairs, vessels, and tattoos. Dr. Sasaki was selected as an innovator, investigator and educator on the SmartLipo TriPlex™ for fat melting and skin tightening during liposculpturing. SmartLipo Triplex™ is performed only by Dr. Sasaki as an office procedure under local anesthesia. Since 2000, Dr. Sasaki has given over 20 instructional courses at the annual meetings of the American Society for Aesthetic Plastic Surgery, demonstrated this technique in over 15 countries, and published seven peer-reviewed papers and two chapters on this subject. 


Cheek Fat Pad Elevation and Volume Enhancement
Cheek fat pad elevation

Dr. Sasaki is the originator (1994) of this innovative technique to elevate the large triangular fat pad in the face by deeply placed and anchored suture systems. Through a single dot incision within the nasolabial line, one or two of his patented suture systems was passed upward and fixed within the orbital rim and/or passed in a supero-lateral direction and anchored to the deeper layer in front of the ear. This surgery remained one of the few techniques recognized around the world that volumetrically rejuvenates the lower lid-cheek junction to obtain a heart-shaped youthful face. This surgical technique was performed in the office operating suite under local anesthesia or a hospital under general anesthesia with combined procedures. Further volume enhancement after cheek fat pad suspension was obtained by fat grafting.

Since 2000, he has given over 15 instructional courses at the annual meetings of the American Society for Aesthetic Plastic Surgery, demonstrated this technique in over 20 countries, and published seven peer-reviewed papers and two chapters on this subject. His DVD educational video has received a Video Award from the Aesthetic Society and was included in its film library for distribution to its members. SAFE Technique

Since 2007, Dr. Sasaki has advanced energized methods of liposuction in determining their clinical safety and efficacy for the benefits of patients. He is an educator for Powered-Assisted Liposuction (PAL) at the national and international levels in his specialty of plastic surgery.

Dr. Sasaki introduced the the SAFE technique, (Suction, Aspiration, Fat, Equilibration) into his practice over five years ago to provide his patients another safe and effective method of liposuction in addition to power-assisted liposuction techniques.

Dr. Sasaki has expanded its usage in his practice by incorporating portions of the SAFE technique in all of the energized methods of liposuction (Powered-Assisted, and SAFE Technique). The SAFE Technique represents a variation of traditional liposuction in which the surgeon remains in control of the entire process of fat removal by fat Separation, Aspiration, and Fat Equilibration.

Dr. Simeon Wall, Jr. introduced the SAFE technique by principles with the introduction of a special basket-shaped cannula that more efficiently removed fat and reduced the irregularities often observed after traditional liposuction. Dr. Wall has been an excellent mentor in implementing his technique into Dr. Sasaki’s surgical practice.

His efforts will help define their benefits and compared to those obtained from traditional liposuction devices. To this end, he has published four peer-reviewed manuscripts on this subject and was invited to teach his colleagues at the 2010 American Society of Aesthetic Plastic Surgery meeting a course entitled, “Energized Liposuction: Limitations and Benefits.”


Ultherapy: Non-Invasive Ultrasonic Tissue Tightening and Contouring

Dr. Sasaki was the first physician in California (and the fourth in the USA) to implement the only imaged and focused ultrasonic device (Ultherapy, FDA-cleared in September 2009) for multi-levels of tightening and lifting of sagging tissues in the forehead/brow, face and neck. This technology offered a precise non-surgical approach to elevation of tissues in a similar, but not as aggressive manner as performed by traditional plastic surgical procedures. The procedure is performed by Dr. Sasaki’s certified registered nurses under his direct supervision. From an external transducer on the skin surface, all tissue levels from the skin down to bone can be visualized on an adjacent imaging screen. The transducer focuses 95% of the ultrasonic energy to a point less than 1mm in diameter and precisely releases a number of thermal coagulation points firstly on the muscle’s fascial covering and, secondly with another transducer, to the lower level of the reticular dermis of the skin. These pin-point sites of thermal injury result in reorganization and contraction of the collagen fibers to tighten and elevate non-surgically layers that are addressed during surgical procedures. Since September 2009, Dr. Sasaki and his registered nurse have treated over 500 patients with a response-rate (eight out of ten patients) similar to that observed in Europe and the Far East. The procedure is performed usually without topical anesthetics. The clinical results can be observed as early as 6 weeks in some patients (majority of patients will respond by 3 months) and can last as long as 1-2 years.

Dr. Sasaki is a leader in non-invasive methods for tissue tightening. He has been a clinical researcher and educator for a number of FDA-cleared devices, including Ultherapy for precise and controlled ultrasonic heating of skin to the forehead, face/neck and body. Dr. Sasaki to presents his experience to many countries around the world.


Hair Stimulation

Dr. Sasaki is an early user and clinical investigator for alternative methods to stimulate existing hair growth in men and women. With the use of fat injection with Platelet Rich Plasma (PRP) into the patient’s subcutaneous scalp the enriched adipose will be applied in a very fine, homogeneous layer at a rate of approximately 0.1mL per square centimeter of affected scalp in the subcutaneous space to promote hair growth.  With this technique the patient will improve or retard hair loss and patients can see results about 3 months after treatment.

Another treatment options for patients, Dr. Sasaki recommends FDA-approved solution 5% Minoxidil Rogaine® (unknown mechanism) and Finesteride® tablet (5α-reductase enzyme blocker) for hair loss in men, as well as 2% Minoxidil Rogaine® for hair loss in women. Dr. Sasaki also is investigating the safety and effectiveness of MicroNeedling/Platelet-Rich Plasma and MicroNeedling/Adipose-Derived Cells on stimulating hair growth in selected patients who have not responded satisfactorily to the FDA-approved Rogaine and Finesteride. CoolSculpting

In August 2009, Dr. Sasaki introduced CoolSculpting® for the first time in Los Angeles as a unique non-invasive body contouring device. This technology gradually lowers skin-fat temperature by an external cooling paddle to levels that cryolipolysis occurs with eventual fat removal. The procedure may take 1-2 hours to complete, depending on the size and location of fullness. Reduction of resistant fat rolls around the neck, abdomen/hips and back rolls may occur at three months to this comfortable, single treatment procedure without any downtime. Treatments may be repeated as indicated by clinical response. With two CoolSculpting devices your treatment can be maximized by addressing two sites simultaneously. Light Emission Diode (LED) Therapy

Since 2002, Dr. Sasaki has incorporated advanced FDA-approved systems of Light Emission Diodes (LEDs) in his practice to synergize the results of medically-based skin care programs, cellulite treatments, and management of acne.

The Sasaki Advanced Aesthetic Medical Center is one of the few plastic surgical centers in the country that incorporates the most varied and advanced systems of LED devices (2002) to synergize and to improve the results of medically-based skin care programs. These gentle red, yellow, and blue light waves can upgrade and downgrade cellular functions with FDA-approved devices to increase collagen and elastin fibers in skin, stimulate hair growth, as well as reduce skin bacteria associated with acne. These non-laser light waves can also reduce cellulite formation, sun-induced pigmentations, and inflammatory reactions from surgery, laser resurfacing, and PhotoFacial® treatments for skin rejuvenation and acne rosacea. UltraSound and Electrical Stimulation Therapy

Since 2002, Dr. Sasaki has used combined ultrasound and electrical-stimulating energies to improve the results of skin care treatments and to reduce swelling and irregularities in surgical areas.
Ultrasound device

Since 2002, the Sasaki InnoVessence SkinCare Center is one of the few centers in the United States that combines ultrasound and electrical-stimulating energies to enhance the effects of medically-based skin care programs. This innovative technology not only increases the penetration of larger molecules (hydroquinone and kojic acid inhibitors of melanin pigment production) to reduce skin pigmentations, but also stimulates collagen and elastic fiber production in skin. MicroNeedling (Fractional Microdermabrasion)

Modern MicroNeedling techniques began in 2000 with a wheeled device with a pre-set needle depth to rejuvenate aging skin and improve scars. Since 2012, Dr. Sasaki has been a lead investigator in one of the automated MicroNeedling devices and techniques. This device contains thirteen needles whose depth of penetration can be changed from a 0.25 to 2.5mm depth, depending on the clinical concerns that range from skin rejuvenation to deeper scars. Modern MicroNeedling preserves the protective barrier of the epidermis by fractionated needle penetrations, each of which activates and stimulates normal or enhanced cascades of collagen and elastin fibers to rejuvenate aging skin, to reduce of fine and moderate wrinkles, to minimize large pores, restore normal pigment cells for hypopigmented scars, to improve stretch marks and crepey skin, smooth out acne scars and to stimulate hair growth in certain types of alopecia. In addition, each of the thousands of microchannels permits the passage of sophisticated cosmeceuticals, pharmacological, bioactive, and cellular agents that can further stimulate collagen and elastin fibers and encourage the growth of hair follicles. Skin / Facial Fillers

Restylane, Juvederm, Belotero, and Radiesse Fillers

Dr. Sasaki has been an educator and instructor on the indications and usages of most of the current fillers since 2000. He has given seminars on Restylane®, Juvederm™, and Radiesse® fillers for wrinkles, folds, and hand rejuvenation to physicians and registered nurses. His expertise is not only reflective of the simultaneous layering of different filler types to obtain more lasting and aesthetic results but also the combined usages of this layering with Botox®, laser resurfacing, and radiofrequency skin tightening techniques. His highly trained registered nurses are nurse-educators and are skilled in all aspects of the safety and effectiveness of these FDA-approved fillers. Volume Enhancers

Methods of fat harvesting, processing and injection are critical to obtaining a higher percentage of living fat cells. Current research continues in optimizing these factors to increase the chances of fat survival and retention within grafted sites. Dr. Sasaki continues as an educator, investigator, and consultant in this field to advance its usage for the welfare of so many patients. The American Society of Plastic Surgery cautions our members that further research is needed before “stem-cell” preparations can claim any benefits over traditional approaches for fat grafting.  As President-Elect to the Aesthetic Stem Cell Society, Dr. Sasaki and his colleagues are working with our societies and the FDA.

Dr. Sasaki has employed lipografting as a natural fill for enhancing and augmenting facial atrophy and depressions, aged hands and flattened buttocks for years.


BOTOX® Cosmetic

Dr. Sasaki is an educator and instructor on the conventional and advanced usages of Botox Cosmetic®. Both muscle relaxants possess similar mechanisms to weaken functional muscles for wrinkle improvement. He has demonstrated his techniques to physicians and registered nurses at seminars in the advanced areas of lateral brow and nasal tip lifting, reducing the gummy smile and downward turning of the corners of the mouth, and lessening the prominence of the chewing muscle bulges at the angles of the lower jaw. Chemical Peels

A TrichloroAcetic Peel (TCA) is considered an intermediate depth peel that reduces superficial pigmentations and promotes effacement to mild-moderate depths of wrinkle formation. This peel is an effective treatment for actinic and aged skin on the face, neck, décolletage, extremities and hands. The patient undergoes pre-treatment skin preparation for at least six weeks prior to chemical peeling. Please click on Sasaki Innovessence SkinCare Center for more information. After the skin is cleansed of makeup and oils, Dr. Sasaki selects and applies the appropriate concentration of the peel solution in a defined manner to reach the desired skin level to ensure safe healing. The treated skin is gently treated with cool saline gauzes and an LED treatment to reduce the inflammatory effects of the peel. Exfoliation of aged skin and subsequent re-epithelialization of new skin may require 7-10 days, depending on the depth of skin peeling. Post-peel skin redness may resolve by 6-12 weeks. Post-traumatic hyperpigmentation may occur in the pigment-prone patient thereafter and is treated with combination treatments in an effective manner. Oral antibiotic and anti-viral medications are prescribed.

Croton Oil Peel can be tailored to be a light, intermediate or deep depth peel that has the potential to improve superficial and moderate depth pigmentations and smooth out mild-to-severe rhytids formation. At this time, Croton Oil Peels should be confined to the forehead, eyelids, face, and periorbital/perioral rhytids because of the depth of peeling. Dr. Sasaki will select and formulate the appropriate concentrations of Croton Oil Peel that are applied to the cleansed skin. A lighter concentration is employed to the thinner skin of the eyelids; an intermediate concentration is applied to the forehead and face; a higher concentration is used to treat the thicker skin around the mouth. Dr. Sasaki is able to assess the depth of peel by changes in skin texture and color to control the level of peeling that is optimal for safe and effective results. The treated skin is gently washed with cool saline gauzes and covered with a Vaseline ointment to ensure rapid healing. Since Croton Oil Peels have the potential of achieving a deeper depth of injury, re-epithelialization may require 2-3 weeks. Post-peel skin redness and post-traumatic hyperpigmentation/hypopigmentation changes may take longer to resolve. Skin preparation before peeling and medical skin care treatment after the procedure are essential for a safe and effective outcome. The Croton Oil Peel is indicated for lighter skin individuals because of the potential for post-peel pigmentation changes. Antibiotic and anti-viral medications, as well as analgesics, are prescribed.

Dr. Sasaki is an experienced chemical peeler, who utilizes different levels of chemical injury to obtain specific and safe results as determined by patient’s findings, ethnicities (pigment –induced potential), and goals. Only Dr. Sasaki will perform intermediate and deep chemical peels in his office.


ThermiTight and Thermi-Rase Lifting

ThermiTight is a radio frequency and temperature controlled system that using image guidance to target areas to achieve desired results in the neck area. ThermiRase is performed by precisely disabling the nerves which control the muscles which cause glabella frown lines and forehead wrinkles.


Platelet-Rich Plasma (PRP)

Dr. Sasaki is an early user, clinical investigator and consultant in the use of PRPs in aesthetic procedures such as a cell supplement to fat grafting to sustain its survival, and, in combination with MicroNeedling to stimulate hair growth, and rejuvenate skin. Dr. Sasaki and others are contributing to the few controlled, clinical studies with evidence-based data to determine the safety and effectiveness of PRPs. He has presented his experience in over two-hundred and fifty patients with PRPs and fat grafting at national and international symposia for the past four years demonstrating improved retention rates in aesthetic surgery. Dr. Sasaki has been an early user, investigator and consultant with four device technology companies in this dynamic field for the past five years. Dr. Sasaki is able to obtain Adipose-Derived Cells by Mechanical Separation (Differential Centrifugation) of these ADRCs from the mature fat cell population obtained from specialized harvesting procedures. Since the clinical use of Enzymatically-Derived Methods to obtain ADRCs has been restricted under current FDA regulations to date, Dr. Sasaki has relied primarily on Mechanical Separation Devices and the use of Platelets, that also contain cytokine growth factors, to improve his fat grafting retention rates. He has presented his clinical experience of over 250 patients at national and international societal symposia. In his practice, ADRC therapy is safe, practical and effective in reconstructive cases such as in poorly healing wounds from chronic infection or radiation injury. In these challenging cases, only cells are injected within the poorly vascularized and healing sites. In aesthetic cases, the ADRCs are usually combined with the processed cleansed mature fat cells and injected for volume augmentation in the hollowed areas of the aging face, thinned and veined hand, volume depleted breasts and buttocks. Fat Grafting for Volumetric Augmentation

Dr. Sasaki is a leader and investigator in fat grafting in the past ten years by optimizing each step of fat grafting. As a consultant to a number of liposuction, platelet technology, and cell companies, Dr. Sasaki has reported his findings of improved retention of fat grafting at national and International Symposia in the past few years. In his AAAA-SF certified surgical center, Dr. Sasaki is able to harvest your desired volume of fat with a syringe or a mechanical device under local tumescent anesthesia in a safe manner and with minimal discomfort. Upon request, Dr. Sasaki will perform a complete liposuction of your donor site(s) to improve your body contouring. The fat is then processed and purified either in a dialysis filtration bag (for larger volume cases such as the breasts or buttocks) or in filtering device (for smaller volume cases such as the face and hands). Both of these processing methods remove significantly undesirable tissue debris, fatty oil, unwanted red blood cells and tumescent anesthetic solution. The processed and cleansed mature fat cells, cells and tissue scaffolding can then be combined with your concentrated blood platelets that contain a large amount of cytokines (growth factors and factors for stimulation of new blood vessels). This combination of minimally injured adult fat cells, cells, tissue scaffolding, and platelets is optimal for fat transplantation by injection for longer survival and retention at your recipient sites.
