Breast Augmentation

An inadequate breast volume alters for some women their self-image and confidence. If your breasts exhibit moderate or significant sagging, a breast lift (mastopexy) may be required along with breast augmentations to realize fuller and more natural appearing breasts. Some patients may select a breast lift without augmentation if they possess sufficient volume or decide against implant usage. 

Using the training from his two-year fellowship in surgical oncology (specializing in breast cancer), Dr. Sasaki will obtain a complete breast history and clarify its significance for yourself and your family. He will obtain any pathology reports you may have from a previous biopsy and interpret the results with you. The doctor will also review your previous mammograms, along with your radiologist, to assess your breast volume and other radiologic characteristics that may influence your procedure. During your initial consultation, Dr. Sasaki will demonstrate how you should perform self-examinations for earlier detection of masses that should be brought to the attention of your physician.

Standard photographs will be taken to provide you with images to be used to discuss more fully your concerns. Dr. Sasaki will encourage you to bring in any other photographs that may demonstrate to him the new look you wish to achieve. During your physical examination, precise anatomical measurements will be obtained to guide Dr. Sasaki on selecting the optimal size and shape of the implants that will compliment your enhanced breasts to your figure.

Breast implants are made of medical grade, biocompatible, textured or smooth shells that are filled with a sterile saline solution at the time of surgery or are already filled with a silicone gel fill. Dr. Sasaki uses only FDA-approved implants. He will discuss in detail the inherent characteristics that distinguish saline from silicone gel implants, that may be round or anatomically shaped or consists of softgel to cohesive gels.  Each type of implant has certain unique advantages and disadvantages about which you must be advised to allow you to make the best informed selection. You should be aware that breast implants are not guaranteed to last a lifetime and may require replacement in the future. Since pregnancy, significant weight fluctuations, or menopause may alter the appearance of your augmented breast, maintenance and regular breast examinations are required to evaluate the integrity of your implants. Breast augmentation requires surgical placement of implants into precise tailored pockets. Dr. Sasaki prefers that the implant be placed beneath the pectoralis muscle (submuscular position) rather than above the muscle (subglandular position). Submuscular placement usually provides more protection for the implant, reduces the formation of capsular contracture (firming of the implant by contraction of its surrounding scar tissue), minimizes implant irregularities, and gives a more natural feel and appearance.

Surgical incisions may be around the lower half of the areola (the pigmented skin surrounding the nipple), within the armpit, or near the crease under the breast. Dr. Sasaki is comfortable with any of these surgical approaches. His decision is based on the type of implant selected, degree of enlargement desired, patient’s anatomy, and scar selection.

A circular dressing, garment, or support bra is placed around your augmented breast immediately after surgery to minimize swelling and support the breasts as they heal. You may shower after the dressings are removed on the third postoperative day. Postoperative directions will be given to you as to the amount of activity you can safely perform. You should expect to plan for an easy lifestyle for three to six weeks and to avoid engaging in any heavy lifting or vigorous exercises.

Breast augmentation may be performed in an accredited outpatient surgical facility or a hospital. The decision for anesthesia is based on the requirements of your specific procedure, considerations of the doctor, and your personal preferences. Dr. Sasaki and his excellent staff will carefully explain your procedure prior to surgery along with specific instructions about diagnostic testing and medications. The results of your breast augmentation are immediate, but final results are observed by three months. Incision lines tend to soften and fade over the next year.
