Neck Lift

If you are tired of the double-chin or are yearning for a distinctive jawline, tightening of loose skin and a contoured neck, a surgical neck lift may provide you with the most aesthetically pleasing and longest-lasting approach to correct these age-dependent findings. Dr. Sasaki will describe the way your loose platysma muscle contributes to the formation of the turkey gobbler presence, jowls, and marionette folds. Most aging necks will benefit by tightening this muscle in front of the neck (corset platysmaplasty) and advancing it back towards the ear and neck hairline, suctioning and contouring the fat layer, and adjusting the loose skin.

A complete neck lift is recommended to address skin laxity, fat accumulations, and looseness to the underlying platysma neck muscle. Since the platysma muscle occupies the entire neck and its extension to the lower third of the face, a complete neck lift will result in an aesthetic improvement at both sites. The platysma muscles in the majority of patients, are fused to one another in the middle of the neck. When muscle separation occurs, each edge of the muscles contributes to the webbing turkey gobbler appearance. When the platysma muscle becomes lax in the lateral aspect of the neck, its looseness produces further irregularities. As the muscle drapes in the lower third of the face, the marionette folds and jowls are produced and accentuated by excess fat.

A complete neck lift usually requires three incisions to gain access to the loose platysma muscles and fatty layers. One of the two identical incisions starts at the crease line in front of the ear lobe, then hugs the back of the ear lobe and ear, and finally extends a short distance to the posterior hairline. The third incision is a short horizontal incision under the chin to expose the separated platysmal bands and fat pad. The entire skin envelope is elevated carefully from the underlying muscle and fat from side to side, as well as a short distance in front of the ear. Liposuction is selectively performed, leaving behind a contoured amount of fat to provide an aesthetic fill. The platysma muscles are drawn together by sutures in the middle of the neck to eliminate the turkey waddle deformity and to gain back the angled look. From the outside ear exposures, the same muscle in both the lower third of the face and lateral neck is appropriately advanced backward and tightened with a number of sutures. This muscle tightening maneuver significantly defines the face and neck with elimination of the marionette folds and jowls. The skin is the last structure that is gently tailored back to its opposite side to obtain a rejuvenated and defined neck.

In selected patients, Dr. Sasaki may suggest the use of Ultherapy, a non-surgical device, to tighten and lift mild-to-moderate degrees of skin and platysma muscle sagging instead of a neck lift. Ultherapy focuses a series of focused thermal coagulation points on the surface of the platysma muscle and the lower level of the skin in an imaged screen to denature collagen and elastin fibers for eventual contraction of these tissues. The doctor may also suggest the use of Thermi-Tight, a minimally invasive radiofrequency device that thermally simultaneously reduces layers of fat and to tighten skin. Dr. Sasaki will indicate candidates for this treatment based on his findings and desires not to have a surgical procedure. Ultherapy will not affect excessive fat collection or bring together the separated platysmal bands in the midline of the neck.
