Your eyes communicate with the outside world. Excess skin folds, undesired bulges, or drooping of the upper lids close to your pupils make you look tired and older than you feel. If you present with extra folds of skin above your double lid line, a conservative ellipse of skin may be removed to give you a refined look. If you desire to have a more defined appearance, the doctor may suggest that both a strip of skin and underlying muscle be taken away. If you demonstrate fat bulging at the inner corner of your eyelid, a conservative removal of fat will enhance your final result. Some patients exhibit too large a bulge (hooding) under each brow that represents a separate anatomic area from that of excess skin and fat which are confined to the upper lid proper. Hooding may be optimally addressed by brow procedures rather than an upper lid blepharoplasty.

If you demonstrate a dropping of your eyelid towards your pupil (ptosis), Dr. Sasaki will perform a series of maneuvers to diagnose the causes for this finding. He will provide you precise surgical options to raise your upper lid from either an outside approach (tightening the voluntary elevator muscles) or an inside approach (shortening the involuntary elevator muscle).

Based on your physical findings and goals, Dr. Sasaki will make specific suggestions to provide you with age-appropriate youthful appearance. Because of his extensive experience, Dr. Sasaki can improve the aesthetic requirements around the upper eyelid areas of different ethnicities.

A well-designed upper lid blepharoplasty is performed through an elliptically-shaped incision, extending from the inner aspect of the lid to the outer aspect of the double lid line. Based on your preoperative discussion, the doctor will determine the exact layers (skin and/or muscle) and fat bulges to be removed to provide you a natural and beautiful appearance. Through this same exposure, sculpturing of excess brow fat can be easily accomplished to give you a contoured look to both your upper lid and brow. Tightening or shortening of the deeper inefficient muscle apparatus (that elevates the upper lid) corrects upper lid ptosis and can be added to a cosmetic blepharoplasty procedure.

Upper lid blepharoplasties may take one-one and one half hours to perform and can be done safely under local anesthesia in our office surgical suite. You may shower and wash you face daily after surgery. Antibiotic ointment is applied twice daily over the incision site. The single buried suture is removed about ten days after surgery.

Crepy skin in the upper lid will not be significantly improved after an upper lid blepharoplasty. The fine wrinkling is a result of excess skin thinning from aging/sun damage and the attachments of the underlying orbicularis oculi muscle to the thin skin. The use of a fractional laser, light/intermediate chemical peels, or microneedling can result in effacement of the crepy skin wrinkling after sufficient time has permitted surgical tissue healing.
