Surgical Facial Augmentation

Some patients elect to have a facelift to improve the appearance of their face and combat the signs of aging. A facelift addresses the loss of volume in the face by repositioning the cheek fat upward and outward to reestablish a more youthful, triangulated appearance with fuller and rounder cheeks and tightened prejowl and jowl areas. The procedure that helps correct these aging signs is the SMAS (superficial muscular-fascial system) facelift with malar fat pad elevation. Some patients, however, may have deficient malar fat pads that, upon elevation, fail to provide a substantial fill to the midface. This type of patients may benefit from facial fat grafting to increase the volume of their cheeks and fill in hollows.

Cheek Implants and Other Options

Other patients, who desire a more permanent method of augmentation for deficient cheeks, may benefit from the usage of a silicone cheek implant (usually inserted through an opening inside the mouth) or microfat grafting (patient’s own fat is harvested from the abdomen or thighs). This may also be considered for smaller areas of volume deficiencies such as hollowness to the lower lid, nasolabial and marionette crease lines and deeper facial wrinkles. A volumetric fill with an anatomically-shaped silicone implant on the bone provides a permanent fill.

Microfat grafting may be injected deeply just over the bone in selected facial areas with different volumes and at variable tissue levels. It is estimated that only about 50% of the injected fat will survive.

For patients who are not ready for cheek implants or fat grafting, Dr. Sasaki will discuss in detail other nonsurgical temporary options such as the deep injections of Radiesse® (Calcium-based microspheres) or Voluma® XC.
